Looks bad.
High speed and no seatbelt invariably results in a fatality.
If they're lucky enough to survive it is challenging to put them back together again.

A very close pal of mine was involved in an horrific crash many years ago. He was in the back and not wearing a belt, when the car rolled he was ejected through the rear window and flung high into the air. When he landed he crushed the lower part of his spine damaging the nerves. He is now in a wheelchair for the rest of his life.
After the crash the police carried out their accident investigation and found that if he had been wearing a belt he would be dead. After he was ejected the car carried on rolling which in turn crushed the rear part of the roof through the rear seat. In their words nobody could of survived if they were in the back of the car when the roof caved in through the seat.
The insurance company even tried to use the fact he wasn't belted to escape paying him out but fortunately they were not successful and he ended up with well over a million pounds.
However, I do agree, in nearly all cases a seat belt will save your life. For a couple of years after his crash I refused to wear a belt but I now always belt up as do the rest of my family.

I certainly wouldn't race a car without wearing a nice tight full harness system. On the road, almost always, belt wearing is a good thing. You'd never be able to see what was coming anyway, so it's all down to chance.
I don't worry about it too much if driving early (pre 70's) and expensive classic cars, Astons, Ferraris, Mustangs etc as if I was careless enough to crash the car, I'd just as soon die with it thanks.

I was fined for not wearing a seat belt a few years back, whenever the wife asks me why I'm not wearing one (when I've forgotten to set off with it on rushing about) I say I'm getting my monies worth.
