It depends which MSA licence you are looking at Phil. To obtain a race licence you have to pass a medical then atend an ARDS training course after buying a starter pack. Once you pass the ARDS course you then qualify for a race licence at national level.
To obtain a rally licence you have to attend a similar course at a rally school, but there is no medical requirement.
Some licence types such as for sprinting/hillclimbing dont require any training course atendance.

I am happy to enjoy the car on the occasional track day
I just wondered how you[General not personal] obtained one as it allowed you entrance to the Autosport show on trade days

Test for National 'B' licence is pretty easy - the ARDS test involves a few circuits (at reasonable speed ) of the track (in my case Silverstone) followed by a written test (basically knowledge of the flags used, plus some 'common sense' questions) and a brief medical.
New licence holders have then to display a black X on the back of their car for their first 10 races.
To renew the licence costs c£40 annually, plus a medical if you're getting on a bit!
One of the more expensive ways to get into the show I would have thought!

You can get a National "B" licence simply by filling in the MSA application form and sending off your £40 without any driving, tests, medical, etc, if you are competing in speed events such as hillclimbs and sprints and are not racing, i.e. circuit racing. If you intend circuit racing then you do need to do as Huge advises.
If you haven't done a hillclimb in your Scooby do it - its great fun and you will really get the motorsport bug !.
