Was stuck in a jam on the M40 last night due to an accident - had not long happened (fortunately nobody seemed to have been hurt), and only 1 plain-clothes old bill was on the scene, and put out some cones. Seemed that the rear offside wheel has fallen off a Corsa and hit a couple of cars before getting jammed under a Vectra (IIRC). Could have been a lot worse.
However, the Vectra driver stopped their car in lane 2. Why?? The 4 other vehicles involved, even the 3-wheeled Corsa, all managed to get onto the hard shoulder. Surely the hard-shoulder would have been a better place to stop than lane 2? Even for self-preservation?
