If you have kids then suspect them 
if not then make a complaint with apple customer services
Ages ago I had 1 click purchasing set up in the mac in the office. A couple of mates came around after work hours for a few beers and whilst sat there mucking about, one of them decided to build their dream mac setup on my mac in the Apple store. I only found out the next day when I got an email from Apple saying my credit card had been refused for the £12,000 order !!!

My brother had his iTunes account hacked. They bought £700 worth of £10 iTunes vouchers.
His bank gave him his money back quite easily. iTunes people were a bit of a sod to deal with but it got sorted in the end.
