The two latter it should only be done as the last resort, the prior (Phimosis) does require it if it is apparent that its an ongoing issue.
The official statistic is 1 to 1.5% of boys have it, which shows Phimosis is an extremely rare condition.
However its quite possible that through a combination of outdated habits/misdiagnosis and generally accepted medical practices/routines its done alot more frequently "just incase" or to "avoid future complications" etc.
There is also the issue of how much skin should be removed, too much causes problems during adulthood/puberty, and also extension of hair up the shaft of the ***** (as it originally was skin from the pubic/******* area thats been stretched up the *****)

Angela Merkel's spokesman has promised Germany's Jewish and Muslim communities they will be free to carry out circumcision on young boys, despite a court ban that has raised concerns about religious freedom.
The government said it would find a way around a ban imposed by a court in Cologne in June as a matter of urgency. "For everyone in the government it is absolutely clear that we want to have Jewish and Muslim religious life in Germany," said Merkel's spokesman, Steffen Seibert. "Circumcision carried out in a responsible manner must be possible in this country without punishment."

IMO like many others have said here, circumcision shouldn't be performed without one's consent. I know of the cases where adult men have gone and got it done. They were not essentially all Muslim or Jewish geezers. It floated their boat, so all cool. But little kids shouldn't be put through this because adults in their family think that it would be better for them. Leaving the foreskin intact is not life threatening, so there isn't an absolute necessity to get one circumcised with or without any excuse; religious or hygiene related. I have been to some circumcision parties where everyone was making merry while the poor kid in new clothing was brought out in his father arms, all limp and anguished. He hardly stayed on with the invitees to have fun, instead he went to bed for the day. I found it ridiculous to eat feast and have a laugh with others while the main hero of the event looked distressed and was absent from the celebrations. I haven't had chance to talk to those kids (adults now) since then. Perhaps they will stick up for circumcision in favour of their religion and hygiene etc, and perhaps they have had their kids circumcised too. I don't know. But my view is that the choice should be of an individual who has reached the maturity to make his own decision. It shouldn't be performed on the babies and young children, just because their parents want it done. Unless there are medical reasons, of course.
