Default Expensive problems?

I'm having a couple of problems with my car. After looking around the forum I'm not too sure what to do next.

Firstly my car is jumping out of 4th ever now and then. When it does happen it is mainly straight away when put into gear. I've read that this happens sometimes with fifth gear and can be fixed by tightning some nut. Could this help with 4th or is it more likely to be the synchromesh? It's just had a new clutch.

Secondly if I rev the car in neutral the is a rattle when I ease off. The noise is still happens with the clutch in. I've read a lot about bottom end bearings going. Is there a way of checking if that is the problem? The noise seems no different if the car is hot or cold. The noise seems to be coming from around the middle of the engine. Just had new cam belt and tensioners.

Any info would be great. The car has done over 80,000 miles so I realise a re build might be the way forward.
