I used to drive one as a sixteen year old, no driving licence (or FLT training) and they let me use a couple of tonnes of motorised equipment with spikes on the front, it was great when the shiny warehouse floor got wet, could do all sorts of tricks, had a very well practised J turn !
However,
Hit a piece of wood from a pallet and nearly turned it over
knocked a breeze block out of a wall
impaled a case of engineering equipment, hastily repaired it and sent it out (to Australia) for Renolds chains despite the oil leaking from it.
The end came when my colleague who was little older or wiser than me skewered the Boss's brand new Orion Ghia injection, right into the boot, both blades, then made it worse by panicing when withdrawing and making a right mess of it

When I worked for an aircraft servicing company it was quite common for us to do repairs to fork holes around cargo doors where some muppet loading the aircraft hadn't quite got it high enough and punctured the aircraft!

Puts me in mind of my first drive of a dumper truck on the local steelworks, as a student labourer.
Foreman asked us who could drive, ie: had a license for the road, and there was only me. He asked me if I could drive a dumper, the front tipping smaller variety, and, not wanting to seem stupid, I agreed that I could
Got it started, drove off, turned the wheel, and let it run through my hands to straighten up...................they're rear wheel steering, aren't they, so it didn't straighten up, it just carried on turning
Demolished a brick wall meant to hide the refuse bins, and squashed 2 bins
Foreman just covered his face..............I think his opinion of students had just been confirmed

Use to be a FLT driver and safety officer at Kodak many moons ago
Any new trucks brought in I was the first to evaluate them
Had around 15 Licenses covering them all reach, counter balance, clamp, stand up ones sit down ones list goes on and on
Didnt get many accidents but when we did it was pretty nasty

Lad at my place took a 1.3 ton pallecon of Golden Syrup off a truck and forgot to put the tilt back. The pallecon promtly slipped off the forks and deposited the entire contents over our carpark Believe it or not - the 600 and odd kg that we didn't manage to save, fitted down your average drain (took about 3 hours like )

If it's the one I think it is, it shows you what not to do with a FLT. I've got a copy which I show during training where this newly qualified driver causes havoc in a warehouse, people's arms getting hacked off, knives falling on heads etc.

I did the course on driving many years ago, and did some work moving pallets, etc,
my boss decided he would run a pallet of spuds into another barn and shot off down the yard, turn sharp right, and went to enter the small barn.
cept the truck would only fit if the forks were on the deck and backtilted..
cue one forklift doing a stoppie!!!! the force was so great, it moved the oak frame out of the walls, and it was a huge frame 12x12 beams!!
my boss was well sheepish for days afterwards...
