I'll start
Last night on the M25 - raining pretty hard. Bloke in a silver 996 911 stops just alongside in the traffic. His rear tyres were so worn, you could clearly see the wear indicators Death wish

Chap driving along in his Golf, with a 7ft wide plywood board on the roof of his car, held on by string which you could use as shoelaces in an emergency, his right arm out of the window, stopping the board launching itself into the air and his mobile phone tucked under left ear whilst he was talking to whoever, with all the above going on at 55mph.

Saxo Boy -- nope, I've seen pensioners picknicking just off hard shoulders before. DRuddle beat me to it!
Worst thing I've seen: being in a mate's Opel Ascona when it lost the NSR wheel at about 75mph (loose nuts ). By a miracle, it didn't roll ... but stopped facing the oncoming traffic in Lane 1. I will NEVER forget the sight of an artic locking up as it was bearing down on us ... but it missed

On the M3 a few years back. heading South(?) towards J3 Lightwater. Somewhere between J2 & J3 there used to be a gap in the central reservation...
Someone on the North(?) bound carriageway decided to turn right ... so through the gap.... straight into the outside lane right in front of me without stopping/looking/ or even indicating and just kept going..... which left me and the guy in the car next to me rubbing our eyes in total disbelief... talk about U-Turn...

I've seen someone driving the wrong way down an off ramp a couple of times
The incident that sticks in my mind though was a good few years ago when the Commonwealth Games were in Edinburgh and there was a coach stopped on the hard shoulder. Upon passing the coach, revealed on the other side was an entire Commonwealth team, with lovely matching tracksuits etc, practicing their running up and down the hard shoulder!

Young couple on the M6 on the way up to Scotland. Running in and out of the traffic to collect their clothes that had exploded out of their roof box. If this wasn't bad enough their little boy was left unattended stood on the hard shoulder!

Driving back on the M1 at night, doing about 90 in a 5-series.
Suddenly a tyre appeared out of no-where and ran right across the outside lane!
just ranover it before I could react to break / swerve. Never did see the lorry that it must have came from!

By far the scariest thing I’ve seen is a caravan being driven at speed.
It was being hauled along so quickly it was flying everywhere. It was at an angle of 45 degrees hopping from one wheel to the other!!!
When I thought things couldn’t get any worse the rear offside wheel hit a road sign in the central reservation, which sent a speed limit sign spinning at a huge speed across the other carriageway (Oddjob style)! Thankfully nobody was there as it was just about head height. Would have been messy
The most recent was last week when I saw a bloke on the M25 with one of those pedal start 3cc moped things. Pecker.

Me, falling asleep at the wheel of a 7.5-tonne truck, empty with a refrigerating compressor mounted up the top, slewing into the gravel trap as we were on the hard shoulder due to lane diversions. The gravel slowed the L/H wheels down causing me to slew further left. I blinked awake, yanked the wheel to the right causing the truck to veer and thus setting up a pendulum effect due to the 1-ton weight of the compressor above the cab, all in a narrower than usual motorway lane with a fair bit of traffic around (lunchtime, M25). I must have given heart attacks to all around, not least myself, but nothing was damaged and no-one was injured.
Am NOT proud of it.

Many years ago, at night, driving uop the M1 and seeing someone headed towards us in the fast lane!
He hit the car three in front of me, I went across two lanes to the hard shoulder and so did the guy in front of me. We phoned it in and two more cars hit while we were on the phone.
Strangly enough no one was hurt.
The silly git had gone onto the motorway on the off ramp and thought he was heading north in the slow lane when in reallity he was heading south in the fast lane.

i saw a guy driving along one day in a golf with what must have been a 70ft sailing mast on his roof on his phone in the fast lane and the mast was moving and causing the car to swerve all over the road. so of course i did the sensible thing and dropped a cog and undertook him.

Cheating a bit - A1 northbound just north of York.
Approaching side road turn of - me about to overtake a 32 tonner when old woman in metro (looked like a retired district nurse) pulls out infront of truck. The truck was within about 50 yards and doing cica 65. trucker applies brakes as there are a number of vehicles all around and he can only try to stop. Truck starts to snake across both lanes and he tries to stop it from jack knifing. Somehow he manages to keep the truck on the road and doesn't hit anything (came within about a foot of the tw@t in the metro) - best bit of skill I have ever witnessed.
Old bat in Metro was NOT Aware of anything that was happening - when the truck had calmed down and I had passed she was just driving along without a care in the world
