Default Crank Pulley

My car has just started to make a squeaking noise which developed into a kinda rattle. I opened the bonnet to find the Crank Pulley vibrating and the front belt had slipped off by one notch. The bolt was loose. ( I havent heard of this before??)
I have tightened it up but still get a wobble on the pulley like it is off center.

I had the all Belts changed a year ago.

Any Ideas??

Thanks

Could be that the pulley rattling round on the bolt has elongated the hole in the centre of the pulley.

ensure you have lined it up and slotted it back into the woodruff key correctly and put some loctite on the bolt when refitting.
