Ive got an online application form that needs replacing. Just a few simple text boxes, fours text boxes, one drop down and one check box. On submission, the details are then sent to two different email addresses.
Thats it basically. Also need all fields to be completed before it can be submitted.
Does that make sense? I can PM the rest.

i write my own but we did look at this and it was pretty good.
