Anyone good with SharePoint permissions?
.. as it appears I am not 
Does anybody happen to know if it's possible to restrict permissions in such a way that only the creator of a document plus another group (say managers) can open it but nobody else?

For anyone who is interested I've sussed it - turn on content approval for the document library. That way, users can only see their own documents until such a time that a member of the approvers group, or someone with full control permissions approves the document for public viewing.
