Don't know about Exchange but SpamBayes is an excellent plug-in I use with Outlook.

GFI ME works nicely - I like to use it in Gateway mode to pre-screen and keep your Exchange server free of bolt-on software. Go for the bundle with GFI Mail Security and you have a nice gateway to keep most of the crap out of your Exchange box.
As Puff mentions, Ex 2003 Server has AS functions. At present it relies on having OL 2003 client side (requires the new caching mode IIRC). A version is in the pipeline to move it server side but I'm unsure if non MOLP SA customers will get it as a free download/add-on.
