User 'tick form' that remebers selecitons next time

Does anyone know how I could have a form, on a web site, that a team would use to tick off docs read, and the ticks would then be remembered next time they came to it.
I have a team of 15 people doing a project (for an MSc). We need to keep track of docs that people have read, so it would need to cater for 15 "users".
Any suggestions, javascript e.t.c. most welcome

You need some persistent session management for that, as in a cookie. Each time a doc is read, set another cookie. I'd serialise a data structure and store that, YMMV.
