......get phone numbers off a SIM card once the airtime provider has de-registered it?
I left work last Friday and had to hand back my phone but managed to hold onto the SIM but now I can't get past the front screen on the phone (albeit, it is a cr@ppy Motorola Timeport)

Depends what the network has done to the card. If it doesn't work in any phone, then you have to get it off with a sim card reader. I believe carphonewarehouse has one of these.

How do you want the data? CSV? Text file? Access database?
Send it up/down/across to me if you want - send it to me at work, cos its their kit that I'll be using

I'll take whatever format that's going to be easiest to pump into the new phone - probably via IR using the Nokia data suite. If that program can import data, I guess .txt would be easiest?
