first signs of brake pad wear?

what are the first usual signs of this? i've been getting a kind of rough lumpy/crunchy feel on my foot when the pedal is pushed down hard near the floor. what is this likely to be. pads discs and fluid was changed on car about 18mths ago, car done 8k miles in this time on normal road use only. car is MY99 uk, 4 pot standard pads n disc brakes.
i'm no expert as you can probably tell, so good advice will be warmly welcomed.

pads usually have those metal prongs on them so that when they get really low the metal prongs squeek against the disc letting you know they need changing soon
as for the lumpy/crunchy feel

Er, it's hard to describe but I guess it's a faint vibration through the pedal and just a noise inside the car - first time it happens is always accompanied by a 'b*gger, got to fork out for new DS2500's soon!'
I did look at the pads the other day when the wheels were being swopped to even out tyre wear and the pads are on about 3mm now so need to keep an eye on them.

I get the 'gravel' feeling when car is first started up in winter. Must get salt on them off the roads. After applying the brakes a few times it cleans the discs up and all is okay.

The 'rumble' noise I get when applying (longish) moderate braking from high speed, usually not instantly noticeable, ~ 2 seconds in then builds in intensity until you back off the pedal if IYSWIM..?
