Get a set of plain discs.
Mark out a series of holes, in a curved formation, then drill them.
3-4 holes per curve, at 8-10mm, depending on the depth of the contact face.
You'll need a pillar drill and a decent clamp to hold the disc.
Thats all they do.

Are you sure? I thought that the holes were cast in and then just cleaned up by machine, otherwise you got too much heat stress around the holes, leading to the discs cracking and thus not being very good at stopping the car...

Holes are not cast.
If you look closley through the hole or down the vent, sometimes you'll see the drilled hole goes straight through a web.
They are drilled either as a straight line or an arc, then de-burred (with chamfer). This reduces the chance of stress cracks on the friction face.
Over a period of time the holes do elongate, as it wears away the material.
Don't forget the disc they start with, is made of a higher grade material.
If your gonna have a go yourself, start off with good quality GpN plain discs, from somewhere like Rally Design, who supply Bremdo, Black Diamond, etc.
