hi either the wheel bearings badly worn, the thread on the cv joint is worn or the nut is worn,
i would start off with jacking it up and checking for play and ordering a new nut mate

As above more than likely the nylock nut is worn or has overheated and the nylons melted,
What ever happened to the castlenut and split pin lol

Did you knock the nut tab over with a punch?
If you didnt it will come lose again, a very common problem.
