1) Lift out the electric window controls by pulling the unit upwards from the front
2) Disconnect the electrical connection from the back of this. There's a tab holding it in...
3) Remove the small blanking panel (about 1.5cm x 1.5cm)
4) Pull off the door handle surround by carefully pulling various points
5) Remove the screw revealed when you removed the window controls
6) Remove the screw revealed when removing blanking panel
DO NOT REMOVE THE SCREW IN THE DOOR HANDLE
7)Remove the popper at the front of the door
8) Remove the triangle of plastic behind the wing mirrors by gently pulling it off
9) Unpop door from the rear and bottom by pulling. If something doesn't want to come, stop and check for more screws and poppers!
10) Once card is loose, pull whole card upwards.

The various dent removal companies can remove dents by poking things down by the glass - I'd leave dent removal to the professionals. They usually charge about £50 which could be worth it as removing the panel is a fair bit of hassle and the chance of damage is high (plastic not too good quality).
