New Registration - Help!!!

Excuse my ignorance etc. etc...... 
What do I need to do to change my registration? Thought it would be easy, but this thing they've sent me is in a foreign language. FFS I'm a sales person, I can't be expected to READ stuff!!!
Have all my documents. Idiots guide?? Anyone done this?

You will need:
V5 showing current registration mark
MOT certificate (if applicable)
DVLA V750/1 (if private plates purchased from DVLA) or transfer form from local vehicle licensing office - look in phone book, and £80.00 transfer fee..........should take approx 5-10 days to transfer.
