Has anyone done this for his or her company? My boss wants me to create one for our office (7 people). What do I include?? Are there forms I can download from the Internet? Has anyone done one that I can modify? Any help much appreciated.

you don't need specific forms - what you need to do is look at everything you need to do to get the office up and running under a number of circumstances. The best and most expensive is to have a shadow office. This will be another office kitted out in a similar fashion with servers mirroring the actions of your active servers. The cheapest option is having a plan where everyone works from home using a pen and paper. You job is to decide what disasters you wnat to protect against and how much money and effort it is worth, document this out and have the necessary bits in place.

Speak to the insurance company or brokers that look after your commercial insurance. I have found them to be quite helpful in previous cases with customers of mine.
The insurance companies seem to be quite proactive about it as a little thought can save them lots of money...

Wow, after all these years of watching people chatting about PC techie things and then someone creates a topic on what I do for a living.
There are a few sites to check out....try www.drj.com
This is the Disaster Recovery Journal, a bit Yankified but it has a 'new to DR' section with hints tips and sample plans. If you are prompted for a userid and password try drj (id) and world (password)
Otherwise try www.globalcontinuity.com for general info...and jobs also www.survive.com
If I can be of any other assistance just shout up.

Jer...just noticed your location.
Comdisco (or Sungard as they are shortly to be) have a large recovery centre just in Livingston. they have office space with PC's, telephones, desks, etc, etc. The only problem is that they sell in units of 25 but they might be willing to negotiate. If you need any contact details you should be able to find them on www.comdisco.com
