Default chainsaw warning

I was out with my chainsaw yesterdya chopping down a tree (from the base, so didn't have the following problem....).

What shocks me is:
1) The guy didn't have it secured to something, such tied to another branch while working.
2) He used a saw without a "dead man" / drop device whilst up a tree
3) His wife worked underneath him, even if he hadn't of dropped the saw, I would have thought there would have been a good chance of getting hit by a branch

There was nothing he could do.
Yes there was Ollyk has already listed 3 things. More common to drop the tool and then try to catch it, bye bye fingers.

He shouldn't have been using a chain saw while on a ladder at all and he must have held the throttle as he came down with it otherwise the chain normally stops moving.
Very sad.

In fact, thinking about it, it sounds a bit suspect. As powerful as chainsaws are it had to be a really freaky accident to have decapitated his wife.
