Hello
I need to "design" (ie crib) an accident report and investigation form (may be 2 seperate forms). would be a great help if someone could send me the ones they use, or that are used in their workplace.
i don't mean the accident book, btw - i mean the forms that are filled in afterwards: failure in safe system, underlying cause, time lost etc etc.

. i don't really think that asking for a couple of forms is exactly taking the p*ss, is it? i am quite happy to help other 'netters with bits and peices. as for "so i don't have to do my job either" - well if all i had to do all day was design forms, i wouldn't be where i am. now have you got anything useful to say?
