Do you have to pay import duty on second hand goods from the states?

Yes, the fact that they're s/h makes no difference. You will be charged duty at the applicable rate, plus VAT at 17.5%, all on top of the total value including shipping cost.

Of course, with any s/h item, its true value is open to a certain degree of interpretation...

When marked as a gift (ie: not commercial) the value dictates whether duty is levied (£30?); for commercial imports the threshold is £18 if I remember correctly.
