Call T-mobile, request a PAC code, give that PAC code to Vodafone and they will arrange the transfer.
PAC codes are valid for, I think, 30 days.
T-mobile won't give you a PAC code if there is money owed on the account of if unexpired contract remains. However, you may be able to negotiate with them to buy yourself out of the contract.

You may have to wait till your contract reaches 11 months, then write a cancellation letter and request your P.A.C. No. When you join voda give the P.A.C no to them it should take about a week.
