Sparrow has it nailed........local into finger (ring block and site) cut off with wire cutters at skin penetration (so you don't drag through external dirt if poss) , dental turbine to cut through nail and pull through
All a bit boring for Casualty on BBC....

Barbed or barbless. If the latter just pull it out, if the former then as mentioned cut hole in nail and pass it through. My cousin did something similar and that's what they did at A&E though I don't think his went through the nail.
