Alternator / p.steering belt keeps coming off

Alternator / p.steering belt keeps coming off
Hi, my rb5 decided to shed its drive belt for the alternator & power steering pump yesterday.
I bought a new one this am from the local main dealer, but then after about 3 miles that one got shed as well. the new belt was properly tensioned...is there something else which is causing the belt to come off, end float maybe on a pulley?? has this happened to anyone else here?
