could be an air leak in the pipework. may have split the tube that goes to the actuator so it's not doing it's job now and just running permanent full boost.
could be that the hole has been there for a while and someone played around with the actuator to get it to boost. now you've fix the hole, the actuator is working properly. not advisable to play around with the actuator yourself though.
i'd start by checking all the pipes around the area where you've been.

all depends on what ecu you have. if you have a dawes device fitted and what the actuator is set to.
depending on your ecu, it may be mapped to run 16.
if you have a dawes devices (brass item in the pipe between the inlet manifold and the actuator) that may be set to run 16. this can be wound out and then adjust in till you get it to the level that you want.
or it could be that the actuator has been played with. it adjusts pretty much the same as the dawes device but be carefull if you try to sort it yourself. get it wrong and you'll be over boosting again which will cause damage.
i used to change the actuator on my rs turbo, but they were a bit easier and cheaper to fix than a scooby if it all went pop.
