A popular network stateside is Vonage, most people I've spoken to seem to be happy with it...

It's a fairly deep subject, but you can tie together some pretty flexible phone systems with a PC, two PCI cards and some nifty programming. Google has loads on this topic, or visit the Wiki about Asterisk
To get started, just try mucking about with NetMeeting or something similar. I use a Mac with an iSight and iChatAV which builds real-time audio/video calls on top of AIM. Apple have documented the implimentation and are working with AOL to bring the same features to the PC version of AIM.
Google is your friend as this is really only scratching the surface. Things get fun when you start looking at traffic shaping, Cisco VoIP phones, SIP ROM images, web based management...
