Ok I have a nice big 18" (visible) monitor running at 1280x1024 with the refresh at 85hz.

For some reason, the picture has gone sort of shaky, - it kind of flickers as if was set to 50hz refresh. If I knock the reresh down to 75hz, it seems to get better, but then I find it still flickers too much.

It doesn't happen all the time, but it's irritating when it does. Anyone have any ideas? I was think that perhaps the monitor is on it's last legs or is running at too high a refresh.

Just try all the different refresh rates and it could be your colour setting. Check it is plugged in securely, try downloading a new monitor or graphics driver. Else dunno some experts on here should know.
