Try Goldwave or Soundforge. The former is cheaper and (for my purposes), a lot easier to use. Hope this helps.

There used to be something called Wav2mid but cant find it (easily), I remember using it years ago, most of this stuff is very hit and miss and you'll find it wont separate out the tracks (cant be done!), just get a single midi sequence for all note on/off/pitch etc. info
