Default Personalised number plate application, which part of V5C to send?

As above, I am assigning my private plate to the car, I do understand I have the send the V5C to DVLV together the the plate certificate. I am just confused which part of the V5C I need to send? The whole one? left and right hand side pages? Or just the left hand side, the side with all the info on?
thanks
