Isn't there a spring clip arrangement behind the winder on them? I think you have to push the door card into the door behind the winder and hook a screwdriver in behind it. It's awkward as **** if I remember correctly. Getting it back is just as bad.

The winder bar has a groove into which the clip erm... springs.
You need to push the doorcard away from the handle whilst groping around behind it with a bent bit of paperclip or coat hanger. Sometimes you can peer along the door and see where the 'head' of the clip is as it's this end you need to 'loop' hold of to remove the winder.

And push the door card in and pull the winder out, look down to back of the winder, you'll see the clip. It's horse shoe shaped so hook the looped end with a screwdriver and prise it out, as it's your first time you'll most likely slip, rip the door card, poke your eye out and lose the clip, no really, I am the voice of experience...well apart from poking my eye out, did come quite close though!
When you put it all back together, fit the clip to the winder, fit the winder to the shaft aligning the splines and bash it firmly home!
