Now thats clever.
My bet is the original coin was skimmed to down to the just before the level of the lowest parts (outline of King carlos) , polished (maybe not necc if a good lathe was used) then re-engraved which would be prety straightforward for a hand engraver.
A lot of work just to go and spend it though
