How on earth do you tune shimano gears on a mtb ? ? ?

How on earth do you tune shimano gears on a mtb ? ? ?
ahouldnt this be easy as pie, but it just ends up worse than when i started !!
If the bike is in the middle most gears (ie middle ring at front an back) the bike is fine, until i change gears...
i get the chain rubbing on the front mech if in any of the extremeties (ie :little ring at the front, big ring at the back, an vice versa) also the chain keeps falling off....gonna get the hammer out in a mo, please help !!!!!!!
