NT & IIS - Inetinfo.exe @ 100% CPU Usage

We have an "e-commerce" thingy - bespoke for the booking of couriers. When a client submits a booking order, our Gateway between IIS & our Novell server, accepts the request, sends the response at which point inetinfo.exe goes into overdrive for the rest of time! This causes the whole of IIS to grind along & frustration to myself and clients.
Has been suggested as a denial of service attack, but this only occurs after a response is submitted.
I have un-installed/re-installed/re-configured IIS & Proxy Server (pant! pant!) & still no joy.
The web pages are created on the fly & they use little Javascript applets to submit jobs, obtain quotations & track jobs.
Any ideas anyone?

Option Pack means it's IIS v4.
Daft question time then..
1) Has it ever worked?
2) Has anything been installed or changed on this box recently?
3) Is there plenty of free disk space?
4) How long does the CPU stay @ 100% for?
5) Try submitting a request with only the IIS service running (stop all the Proxy server services, FTP etc etc).

In response...
a) Fine until Friday AM
b) Not in the preceeding few days.
c) Yes - "beaucoup" as they say in France
d) Doesn't stop (drops a bit if you move the mouse, run another program etc, but not below 80% then straight back to 100% when it can...

We had similar problems which we found to be related to the SMTP server that "can" be installed with the Option Pack.
The inetinfo service went to 100% and the smtp service failed to send emails.
If you are using that service to mail the client a confirmation, check that the service is correctly configured. Also if you are using another mailserver as a smarthost that it is running properly.
Oh and upgrade to Win2000. IIS 5 is head and shoulders above IIS 4.
