Door damage - can it be fixed?

I have a hole about the size of a cigarette burn, but with the plastic bit still attached on the top of the passenger door panel. So, if the window was open and you hung your elbow out - about where your arm would sit.
Can this be repaired or will I have to pay a fortune for a whole new door panel?

I am also interested in getting the interiors of the door panels of my '99 turbo refurbished as they have a few minor marks and a couple of areas with slight damage. If anyone can recommend anyone or anywhere that might be able to do this, I'd be grateful.
