hesitation at 4000 rpm?

Most noticable in 5th Gear. If sat at around 4000rpm and then try to accelerate quickly-revs jump to around 5000-5500 before falling back to 4000 and accelerating. It is as if the clutch was dipped.
Anyone know what may be causing this?
Car is a P1.

No,clutch test is usually 4th or 5th at about 30-40mph.
put it in 4th at 40 then floor it ,if revs rise and speed doesnt then clutch is buggered.
listen to the engine note when its under max load

My clutch is on it's way out at the moment and even though it does slip in 3rd - 4th is far worse.
The best thing to do would be to get up to 2000 rpm in 3rd gear and dip the clutch before you raise the revs to 5000 and then feed the clutch back in quickish. If your clutch is spot on the revs will quickly come back down to earth - if they don't drop then prepare for a £700 thump to your wallet
