Default Coasting feeling between gearchange

Hi

When changing from 1st to 2nd on heavy(ish) throttle, it feels like the car 'over revs' between the gearchange...basically like you're coasting on the clutch (even when it's punched to the floor)...also, as I put the clutch in for the first time each morning when approaching the end of my road, the car will jolt forward when the clutch is depressed as if something in the drivetrain is sticking and then releasing...any ideas?
