Clutch linkage problem - help!

After a year of trouble free operation, the clutch linkage in my 1997 STi broke. I have not taken the transmission out yet to inspect the parts, but I wanted to know if anyone has any experience with this problem:

When mating the engine to the gearbox, the clutch slider engages the pressure plate and "clicks" into place. This mechanism appears to have failed. When I operate the clutch fork lever by hand, the slider appears to be moving freely in and out of the pressure plate without cathing it - as if the "clip" mechanism broke. Has anyone seen this happen? How do I keep it from recurring? Is it possible that I assembled the clutch incorrectly the first time? It seemed to work fine for about one year...
