Take HDD out of old PC and put it in new one. Either then a) keep old HDD in there or copy music over to neww HDD.
I'm not sure how much knowledge to assume. Are you happy opening up the PCs?
Do you have the large iPod with ALL your music on? If so, there's software out there that will copy from your iPod to the new machine.
Alternatively, if you have a network at home, have both machines running and copy files between the two.

If itunes is set to 'Let itunes organise my library' then you can simply go to my docs>my music>itunes music and copy everything there to the same directory on the other PC. From memory there is also a directory file (.xml extension) which you dont want to copy as you want itunes to recreate this on the new machine.
Ive even done this going from PC to mac and it worked flawlessly.
