I need to wite a website that does the following.

A user enters several email addresses and assigns these to a named group He also assigns several times throughout the day say 10am, 12pm, 2pm, 4pm

The site then emails these users with a link and they fill out a form, answering a few questions, the DB needs to retain this info

The site then needs to email all these addresses at pre determined times set by the user at step one.

Anyone seen/got/used any code that I could modify/use, preferably PHP/MySQL?

You won't be able to schedule jobs with PHP, unless the script you run under some cron-like device is in PHP. Either way unless you have some back-end access you won't be able to shedule jobs.
