It only seems to happen with my own pictures. If I delete them from Messenger and just leave the original few, then the one I choose stays put. Introduce some of my own again and the random selection at start up starts again.

Is that right Fig? I'm constatnly changing mine cos it make me smile, it's normally a case of, "this is today's ugly person". I've never had any trouble along these lines.
Just as a point of interest have you tried the following;
Resizing the photo to the right size.
Short filenames (8.3)
Is it truly random, or is there some pattern to it?

Not checked for a pattern, just noticed every time I sign on a different picture is displayed, chosen from the ones I have added.
Don't think file names has anything to do with it, according to the help pages any picture can be used, but square ones give the best overall results.

You got any bolt ons for it that might be affecting it?

I don't know if this is any help at all, but I've been scouting round my computer, your display pictures as stored in
C:\Documents and Settings\<USER NAME>\Application Data\Microsoft\MSN Messenger\<MESSENGERID>\UserTile
(XP&2000, I don't know bout Win98, probably similar but in c:\windows\<somethin or other>
Their extensions are .tmp but they are actually png files, renaming them .png will allow them to be viewed.
I'm not sure how it knows which one to display at the mo.
Given that it looks very much like these pictures are held locally, and have a 'generated' filename, I'm guessing that you are using 2 computers, in which case the picture with the file name 'TFR1C2.tmp' is not gonna be the same from one computer to the next.
Reckon I've cracked it actually, just sat here mumbling away to meself
