Scooby looks ok this morning, no bleeds through the night, which is a good sign
I receieved a PM late last night from someone, who has spoke with a Vet he knows in the States
This is what he has said
I for one would not rule out completely a squamous cell tumour high up in the nose as they are very highly vascularised (lotsa blood vessels going to them) and once they stat to bleed it's hell to get them to stop, the tumour is also a great breeding ground for bacteria that is in the nose anyways. Furthermore, SCC can appear at any age, seen it before in a young Golden with exactly the same presentation. Not a good prognosis, sorry. As for Aspergillosis, if it's causing that much bleeding there is a worry about bacteraemia (bacterial infection in the bloodstream) and would need to be on IV antibiotics. On a whole I'd give this case a very guarded prognosis, wouldn't wait for the culture results and throw the pharmacy at the guy, it's expensive yes, but may be his only hope at this point. He can't afford to keep bleeding, the Uni would be able to do a CT scan or MRI of his head and see what exactly is going on. One note further, there's a plate of bone at the back of the nose (cribiform plate) that has holes in it, the holes allow nerves to pass from the brain to the nose tissues (that's for the great sense of smell) and if the tumour or fungus or bacteria has invaded the plate it may also be endangering the brain as well, prognosis very poor if this happens.
Without rads or complete bloodwork to look at it's the best I can offer.

I will be passing it on to my Vet when they phone today, and see what they make of it, and also to check if they have done any other bloods than the Aspergillosis tests

I have to say, there are some gorgeous dogs posted on this thread Thanks everyone
Up to now he seems fine, a little wet stuff coming from his nose but it's more yellowy than bloody, so I am hoping it will stay that way! He has had his tablets too He must think I am awful putting all these tablets down his throat every day

I am fuming at the moment
My Vet has phoned and has spoke with the Consultant in the Animal Medical Centre in Manchester, he has said there is a risk to us all, and more so Donna with her being pregnant, and we are to seek medical advice from our GP
He will treat Scooby and his preferred method is Drilling and the full surgical procedure, and can see him as early as the back end of this week, possibly next week,
The other option is that there is another place near me that may be able to do the treatment, alternatively my own Vet can do the treatment
But she has said there is a risk and we need to speak to the Docs, and also she asked if we have considered having Scooby put to sleep, some of that due to the risks involved
She did say that she really doesn't know and that I should speak to the Consultant and my GP for further advice
I tried to phone the Consultant to be told this ' I am sorry but I have given your Vet all the information that is needed, I will NOT speak to you, unless you book and pay for a consultation'
I am fuming at that, so Donna has phoned the GP, and because she is pregnant, they have told her to phone the Maternity Ward I have now phoned the GP, kicked off a bit at the jobsworth receptionist and now got an appointment for Donna for this afternoon
I have spoke with the Vet again, told her I am really unhappy with the Consultants response, she has apologised as she didnt realise they will not speak to owners etc, so she is phoning the other place near me, and has booked Scooby in for 1.30 today to start his treament there, as they need to start as soon as they can really
The main people from risk are those with low immune systems, and a new born baby will be just that, although not due till December, the Vet did say that Scooby will have had this for some time, and as he is in and out of the house, the Aspergillosis spores could very well be airborne in the house
Things really dont get any easier do they ?

What a lovely pic!!
I am seeing the doc later today to find out more and probably have some tests done, I will see about getting the children checked too. I also think Steve should see his GP and have a check as well.

The vet has mentioned something to Steve about advice n keeping the aspergillosis out of the house, but it's not easy

Ok, Many phone calls later
My Vet has secured Scooby a place in the St Helens Specialist Hospital referral unit, and will be going in tomorrow evening after work, to begin surgical treatment on Thursday
He will be ready to come home on Friday
Now they have warned me that it is only 80% successful and that there is a chance it may happen again, but just that we need to be made aware of the risk
I told her we really dont have much choice, as we cannot leave it untreated, so we have to go ahead, and make sure he is in the 80% bracket
My Vet is organising the proper Tablets today, so he will be on those for 8-10 weeks, and I am to collect more Anti Biotics and Sedatives later this evening
The specialist I spoke to said they couldnt believe the attitude of the Manchester Clinic, and that if there are any questions at all then please ask, and the consultant will answer them

Well Scooby goes in tomorrow evening to the Hospital, and wil be having his procedure on Thursday, and will come back home again on Friday
Since I have been home, he is more or less back to his usual self, and the bleeding has stopped again thankfully

All the bolts came loose on it a while ago due to the constant jumping up and down on it
I did extend it out once and put the middle section in, now THAT was a bad idea
He jumped on it once it tipped over and he used it as a slide

Took Scooby off to the Vet Hospital, what a nightmare he was, he would not settle at all, and jumped, barked, whined at just about everything and everyone, I wasn't in the waiting room long before being 'asked to wait outside till he calmed down' they were nice about it, but I told them he wouldn't calm down and it's because he is on a lead and can't go sniffing around cocking his leg or playing with the other dogs
I got in with the Consultant and Scooby still wouldnt let up at all, so it made it very difficult to speak, or to hear what the Consultant was saying, but 40 mins later I managed to get enough information to grasp what is going to happen
We spoke about the issues Scooby has had, and about his notes from the Vet's and he told me that there are some new drugs now which are cream based and not the thick sticky emulsion that has been used in the past, and he will give me some papers on it on Scooby's discharge as I have read up on aspergillosis and the various treatments
He is going to drill Scooby's head into his Sinuses, and insert the cream, and this will stay in for about 2 weeks, and work its way all through the nasal areas
He is going to take an X-Ray of his Nasal area, one to check the extent of any damage that may have been caused, and two to double confirm it is Aspergillosis, as he said the Serology is a good indicator but not enough on its own, but also the bleeding from the nostril is another good indicator
He said with Aspergillosis it is very difficult to diagnose, and one test alone is never usually enough, but the symptoms along with the tests are the way to do it effectively enough to warrant this kind of invasive treatment
We had a reasonable chat on the various methods of treatment and he did say that this is now pretty much the most effective method and no longer requires the 10 days of Hospitalisation and flushing twice a day (I wish the main Aspergillosis website would update its details lol)
They will keep him in for observations during Thursday evening, and they will phone me tomorrow afternoon once surgery is complete with an update
All being well he will be available for discharge on Friday
This particular method of treatment is £1000.00 plus any drugs he may need, now what they didnt confirm is whether the X-ray is included in that £1000 or not, they did tell me yesterday any drugs that are required will be extra cost, and said he will more than likely start a new course of Anti Biotics upon discharge
I think upto now my own Vets fees have finished, certainly for the time being, but I am to contact them once I get Scooby back, plus there is an outstanding balance of £115 to pay them, which will be cleared on Monday
I will on Friday once I get Scooby back, and the invoices scan them all in and post them on both threads, so everyone can see exactly how much has been spent, and what it has been spent on
I have stopped the fund slightly short of the target as one or two cheques have been sent, and also a number of people have donated a significant amount of money, I didnt want this to continue to happen and go way over the target, as we now know more or the less the total costs, barring any hidden drug/X-ray charges and Scooby making a 100% recovery without any further costs
Everyone has been incredibly generous, and we are so grateful for everyone's help and support at this difficult

OK MAJOR DEVELOPMENT IN SCOOBY
The Consultant has just phoned and he is currently NOT going to perfom the surgery on Scooby at least for the time being
What he has done, is taken some more X-rays of his nasal area to check for any destruction of the tiny bones in the nasal area, as in Aspergillosis cases these usually present themselves as what looks like hole punch holes in the tiny bones (turbinates)
However, he cannot find any trace of this damage or anything else for that matter, so he has currently withdrawn the treatment for the time being
What he then did was a rhinoscope/endoscope to look in depth at the nasal area, and checked the right nostril and everything appeared to be fairly normal
When he checked the left nostril he noticed quite a few lumpy pink looking polyps, or very similar to polyps and upon touching one, he noticed it tried to start a bleed, so he has now found the cause of the nose bleeds
Whilst doing the rhinoscope/endoscope he has taken a few biopsies, and has some very good samples now to be sent off for testing, which will take a few days to get the results back
He is now uncertain that Scooby does have Aspergillosis, but hasnt ruled out the fact that he may well have been exposed to it at some point, due to the serology coming back positive, and as a result if these current findings he feels that drilling two large holes in Scooby's head and pumping it full of anti fungal cream wont be of any benefit to Scooby, and may even cause irritation and possible problems with the polyps
With all this information he said he will more than likely keep Scooby in till Saturday now, but will phone tomorrow at some point to let me know what is happening, and will keep him sedated for tomorrow anyway to keep his blood pressure down
I asked about how this will affect the costs now, and he said that if he does still perform the Aspergillosis Surgery then he will do it for the costs already stated, obviously the polyps are going to have to be treated and I guess that will become more apparent once we know what they are

oh man, I feel for you. I do try and look for the good news. So, he is hoping that the new alternative aliment is going to be a lot more pleasant to treat than the hole drilling for scoobie.
Its a shame the vet did not have the kit to look up his nose before

Well this is the thing, my own Vet doesnt have an endoscope, neither do most Vets, usually the only way they can get an endoscope is by referral
If you think of it like your GP & Hospital, GP can only do so much then has to refer, either because they dont have the equipment or the specialisations
Now previously in my Vets, they did talk about referral but held off, as they said I would be looking at 1500 for a referral, plus whatever treatment he would need on top of that
So my Vet decided Scooby needed treating for Aspergillosis as the blood results come back positive, as they were fairly certain this is what he has got, after phoning Manchester & Liverpool for treatment of Aspergillosis by referral but *only* for treatment not diagnosis, they got the costs and decided this was the way forward
Manchester have serious attitude problems and have lost out on 'my business' for want of a better word, Liverpool are far too busy to take Scooby until another week or so, my Vets were too anxious about doing the procedure themselves, and the Senior Vet is on holiday this week, so they managed to find a Hospital that has done this treatment before, have a very good reputation and are still in my home town
