Update: Got it back, new EGR valve, new inlet manifold, new water pump, new cambelt.
Car now pulls like a train again, and the juddering at 1500rpm in 4th has disappeared. No smoke on acceleration any more and starts first pull.
So...we are back to square 1 with it.
Now...do I get an EGR delete done, in order to stop this happening again?

Depends on if you are keeping it!
EGR and oil breather system is the source of the soot and oil sludge (assuming the turbo oil seals aren't that leaky) that essentially is what killed the manifold.
Although the flap linkages are plastic so they are always going to wear. I'd also be temped to disable the swirl flaps so they are open all the time; that way they won't jam or wear out.
Alternative is to run a Intake cleaner through it every year as preventative maintanence; Just be careful when using it as some stuff I've used has got the downpipe cherry-red!!
