Get retention forms from DVLA and fill in then send off or take to your local DVLA office. Simple as.

As Mr RA said

Their are some points to note that can make easy, awkward or just plain difficult

Easiest way

The car it is comming off, has to Taxed or within 180 days after expiring, a current MOT(if needed), The V5 in your Name and insurance in your Name and current for THAT car. Just like if you were to re-Tax the car

Slightly harder

if written OFF a little bit more awkward than above, you need a letter of "no Intent" and proof of being written off from insurance/accessors etc

Dfficult but can be overcome

starts to get HARD if the car has no MOT or Tax or V5 in someone elses name it gets a bit sticky

IMHO
