Default Crunching Info Forth

Hi Everyone...Can You Help.
Im having a issue with my car.....
When I accelerate just past 5000 revs and change from 3rd to 4th, it crunches before going into forth.
If on the other hand drive at a normal, and change below 5000 revs, it gos in without any fuss or bother.....
Can anyone help or give any advise.
Ive got a 2003 blob eye with 79k on the clock.
