Oil Catch Tank Fitting?

Hi Help needed please! 
I know this has been asked many times in the past and there are threads on here already.... butt none of them seem to maakee sesne to me.
Basicaly i need too fit an oil catch tank but am unsure where the pipes go and where needs to be T'd into?
Some say there are 3 pipes, 1 acts a breather. But i am confused as surely there are just 2 pipes and all you do is make the oil sludge pass into the tank and clean air circulates the other side?
Any with pic's or a decent fitting guide would be most appreciated...

1 on each rocker box and 1 on the crankcase,"T" the rockers together and the c\case straight onto the catch can,Graham 911 on here has done a good diag i will see if i have it..
