As the others have intimated, the person most likely to know what's caused damage that serious is you.
It looks like you removed the air filter and dropped a box of nails into the induction pipe. What's with all that scarring and pitting in the throat of the housing?
Would also be interesting to know how much play there is in the bearings...

Anyway, It looks like something has been caught in front of the blades for a while but where has it gone after that?
Usually when something goes through the blades, it takes several out with it. The picture looks more like something was fixed in front and "milled" the blades and was then pulled out.
I can't think of anything on the induction side that can come loose and fall in.
I take it that something catastorphic happened recently - its not something that you came across by accident while dismantling the turbo?
As it happens, I have a TD04L available that was going in my S/tb. The problem turned out to be the exhaust luckily.
