Default clutch

Was wondering if someone could offer any advice about the clutch in my my00 turbo (41k). When changing gear when the engine is warm as soon as i put my foot on the clutch to change gear the revs rise slightly, some times it feels a bit notchy going in to gear (has a quick shift fitted) does it sound like the clutch needs replacing? if so how much and can anyone recomend somewhere quite local (live in wrexham)
