Default 03 Sti - Need new clutch?

My car has done almost 40k miles now and the clutch is starting to slip (noticable in higher gears @4-5K rpm) but on some days the clutch does not slip at all.

1/ Could the hydraulic clutch fluid need changing or am I just kidding myself and should get a new clutch.

2/ Would I be right in going for an uprated fast road exedy as car is approx 330-340bhp?

3/ Do I need to replace the flywheel or have it skimmed?

4/ Where would you recommend to go for replacement as work between the Midlands and Oxford.
